Nepafenac is a new type of non steroidal antipyretic, analgesic, and anti-inflammatory drug (NSAIDs) used for the treatment of eye pain and inflammation caused by cataract surgery. Compared with traditional NSAIDs, the chemical structure of Nepafenac facilitates its rapid penetration into the cornea and distribution at its target site, thereby helping to reduce the accumulation of drugs on the corneal surface and reducing the occurrence of ocular surface complications. It has advantages such as strong penetration, strong targeting effect, and minimal toxic side effects. On August 19, 2005, the US Food and Drug Administration (FDA) approved the use of Nipafenamide eye suspension for the treatment of cataract surgery related pain and inflammation, making it the first approved eye NSAID prodrug formulation. After ocular administration, Nipafenamide can quickly penetrate the cornea and be converted into aminophenic acid (NSAID) under the action of eye tissue hydrolytic enzymes; And aminofenac exerts its anti-inflammatory and analgesic effects by inhibiting prostaglandin H synthase (cyclooxygenase) and blocking the synthesis of prostaglandins. As is well known, prostaglandins are one of the mediators that cause intraocular inflammation, which can lead to the breakdown of the blood aqueous barrier, vasodilation, increased vascular permeability, and leukocyte chemotaxis. In addition, prostaglandins can also contract the iris sphincter through non cholinergic mechanisms, triggering pupil contraction reactions during and after ophthalmic surgery. After ocular administration, NSAIDs can inhibit the synthesis of prostaglandins in the iris, ciliary body, and conjunctiva, thus preventing eye inflammation and reducing related pain.
