Temozolomide is the first orally effective imidazototetrazine antitumor drug, which belongs to the second generation of alkylating agents with anti-tumor activity, which does not require intrahepatic metabolic activation after oral administration, is characterized by easy penetration of the blood-brain barrier, good tolerance and no superimposed toxicity with other drugs, and has synergistic effect with radiotherapy, and is suitable for malignant gliomas that recur after conventional treatment, such as glioblastoma multiforme or degenerative astrocytoma, and is also the first-line drug for the treatment of metastatic melanoma. Temozolomide was synthesized by Cancer Research in the United Kingdom and later transferred to Schering-Plough in the United States for development. The drug is different from existing antitumor drugs, it has a novel chemical structure, it is a derivative of imidazototetrazine, temozolomide does not act directly, at physiological pH, it is rapidly converted into the active compound MTIC [5-(3-methyltriazole-1-)imidazol-4-amide] by the non-enzymatic path Chemicalbook pathway. It is thought that the cytotoxicity of MTIC is mainly due to its DNA alkylation (methylation), which occurs mainly at the O6 and N7 positions of guanine.
